Impaired Driver Assessment
I am an approved provider by the New York State Office of Alcoholism and Substance Abuse Services' (OASAS) Clinical Screening and Assessment for the Impaired Driving Offender.
This screening and assessment is separate than my regular counseling services. Please contact me to discuss fees and necessary documentation.
What is an assessment?
An assessment is used to gather information about your history of alcohol and (or) drug use, previous treatment, risk for continued use, significant medical and mental health concerns, sources of natural supports and protective factors to assist in positive life changes, environmental factors, employment history, familial and social relationships, and legal history. The assessment is used to determine if treatment is recommended and what level of care is advised.
What is the process?
There are several required components of the Clinical Screening and Assessment:

Signed NYS OASAS IDS (Impaired Driver Services) consent form

A Urine Drug Screen (UDS)

Completed questionnaires and measures

Copies of the arrest report(s)

Information obtained from at least two collateral persons

A face to face meeting
The assessment will be conducted in my private office, and will take approximately 2 to 3 sessions at 45 minutes each to complete. During the assessment, I will ask you for the names of at least two individuals (collaterals) that I may contact to obtain further information. I will reach out to the collaterals listed within two business days.
After the assessment is complete, you will need to go the laboratory for a urine drug screen. The results will be included in your report.
What to bring to session?
Please bring all relevant paperwork. This will include but not limited to:

Signed IDS consent form

BAC (Blood Alcohol Content) results from the breathalyzer. ** Please inform me if you refused the breathalyzer.

Your payment
How much is the cost of the assessment?
My fee for the full assessment plus the necessary paperwork is $350. Payment is due at the time of the assessment. I accept cash, money order and debit. I do not participate with insurance, as many insurance companies do not see this as medically necessary. If requested, I will be happy to provide you with a receipt/superbill so that you can submit it for reimbursement directly to your insurance provider.
Note: The Urine Drug Screen is not included in the fee. This will be an expense paid by you or through your insurance carrier directly to the laboratory.
